Surrender Form
Thank you for surrendering your animal to the HERPShop. We will evaluate your animal for possible placement. Every effort will be made to place your animal in a good home. The animal may be humanly euthanized if health problems are too severe. We cannot answer inquiries as to the disposition or where about of the animal after adoption. Please answer the following questions about the animal to the best of your abilities.
By completing the following form, the owner, listed below, hereby surrenders all claims and ownership of the listed pet to the Herp Shop. The pet shall be given medical attention at our discretion and found a suitable new home and/or be used as an educational ambassador while residing at the Herps Exotic Reptile and Pet Shop.
